As a pain physician, I must say this book should be standard reading in fellowship if not residency too. It does the best job I have ever seen in terms of laying out basic opioid science, the "big picture" of pain management (with a gift for expressing some of the hard-to-verbalize truths we run into in the field in a "non-offensive" way) and providing guidance for the delicate topic of opioid prescription. To top it off, tackling the complex and controversial topic of addiction is done in a thought-provoking, compassionate and yet no-nonsense manner that makes me reconsider a lot of the current medical dogma behind the disease model (which I have begun to think is completely off-base.) I love the "epidemiologic triangle" concept applied to agent (opioid), vector (us) and host - the first chapter is particularly entertaining and historically enlightening in comparing and contrasting the modern-day opioid crisis with historical epidemics.I love the analysis that what separates this epidemic from infectious disease or other chronic Western disease epidemics is an unprecedented pursuit of the agent:"There is a third limiting factor however that isn’t generally considered in most epidemiology courses, perhaps because it is too obvious to us as rational creatures:the desire of the vulnerable population to avoid the agent." The section on Best Practices Education is fantastic, again, with a thorough consideration of evidence (or lack thereof), regulatory and advisory issues pertaining to prescribing, and some bona fide expert advice. This section alone is worth the cost of the book just in terms of education on legal and standard-of-care issues. Again, the section on dependence and addiction ("Host factors") is an added plus, with perspective and insight I really appreciate, being a non-addictionologist, but recognizing the fine line between the subspecialties. All in all, this is a tremendous book and I look forward to digesting it a little more and incorporating its principles not just into practice but literally into conversations with patients.

Do you want to know the factors that created the "perfect storm" of the present opioid epidemic? It took some time to read this book, which I purposefully studied and marked on! Then never having written book review before, struggled writing it. The diversity of context is extremely well organized and well-presented.I am a Family Practice Physician, dealing with the full spectrum of ages and medical issues. I have practiced from the beginning of the opioid epidemic in the 1990's to now. I have experienced the definite pendulum of "not prescribing enough", to my need for re-education. This book is a must read for any prescriber dealing with pain management.The Preface is very concise, and a good reference to go back to. I particularly like the historical references, being a history buff.The title of the book accurate. The CLINICAL and practical guidelines for the Practitioner were intertwined with the EPIDEMIOLOGIC perspective over the last 20 years. The descriptions of the agent/host/vector are quite interesting and applicable. The Chapters were easy to understand, and were in a logical sequence. Understanding the basic science sections such as opioid pharmacology, adverse effects of specific opioids, and the role of receptors are crucial. There are "how-to" sections of starting, weaning, and discontinuing opioid therapy. Treatment goals for both Cancer and Non-Cancer pain are detailed. How did this book change my practice? Approaching the patient as a whole being has not changed. I learned how important risk factor assessment and stratification is. I learned how much I didn't know about addiction, dependence, Opioid Use Disorder and Pain physiology. I utilize the State Prescription Monitoring Program and utilize Pain contracts. Understanding and managing the conditions that contribute to chronic pain states is a priority. I am no longer hesitant to address changes in diet, exercise, smoking cessation, and alternative treatments to opioids. I am not ashamed to offer to pray for patients. I am a Physician on the front lines of the opioid epidemic. I have the powerful weapon prescribing opioids. This book educated me on using them more responsibly. For both the Clinician and the Patient, the Author describes the enemy as the "lack of perceived danger." The danger lies in the lack of education.
